Most interaction advice arrives as a list of things to avoid. That framing is incomplete, and in one important case it’s backwards.
Understanding the three mechanisms behind these interactions makes the advice easier to apply — and explains why the standard rule for one common medication is not “avoid” at all.
Three mechanisms, not one long list
Nearly every supplement-drug interaction works through one of three routes.
Binding in the gut. Two substances physically combine in the digestive tract, forming a complex that can’t be absorbed. Timing solves this — separate them and both get absorbed normally.
Changing metabolism. A supplement speeds up or slows down the liver enzymes that clear a drug. This one cannot be solved by timing, because the effect persists for as long as the supplement is being taken.
Adding to the drug’s effect. The supplement pushes in the same direction as the medication, so the combined effect exceeds what was intended.
Knowing which mechanism applies tells you whether spacing doses fixes the problem or not.
Binding: the four-hour rule
Thyroid medication is the clearest example.
Calcium and iron form insoluble complexes with levothyroxine in the gastrointestinal tract, reducing how much is absorbed[1]. The FDA label explicitly identifies calcium carbonate, ferrous sulfate and certain phosphate binders as requiring four-hour separation[2].
The consequence of ignoring it is specific: persistent hypothyroidism despite an apparently adequate dose[2]. The medication is being taken faithfully; it just isn’t getting in.
Magnesium and aluminium-containing antacids bind by the same mechanism[3]. Coffee is worth mentioning too — taken within 15–30 minutes of the dose, it has been reported to reduce absorption by around 27%[4].
The most common mistake is a multivitamin, because it contains calcium, iron and magnesium in one tablet[3]. Someone carefully avoiding a calcium supplement may be taking calcium anyway.
The practical version: levothyroxine first thing with water, wait, and put the mineral-containing supplements at lunch or dinner instead.
Consistency, not avoidance
Here’s the one I had backwards.
Vitamin K interacts with warfarin — that much is widely known, and the usual conclusion drawn is to avoid vitamin K. The NIH Office of Dietary Supplements states it differently:
People taking warfarin and similar anticoagulants need to maintain a consistent intake of vitamin K from food and supplements, because sudden changes in vitamin K intakes can increase or decrease the anticoagulant effect[5].
Both directions. The problem isn’t vitamin K itself — warfarin dosing is calibrated to whatever your usual intake happens to be. What destabilises anticoagulation is *change*.
Which means suddenly cutting out leafy greens is itself a change, and can shift anticoagulation just as a sudden increase would. Someone who eats spinach regularly and then stops has altered the equation.
The instruction is steadiness, not avoidance.
A second point on anticoagulants. Several supplements add to bleeding risk through their own antiplatelet activity rather than through vitamin K — omega-3, garlic, ginkgo and high-dose vitamin E among them[6][7]. That’s the third mechanism, additive effect, and timing doesn’t help with it either.
Metabolism: the one timing can’t fix
St John’s wort is the dietary supplement with the most documented drug interactions[6].
The mechanism is well characterised. It contains hyperforin, which has strong affinity for the pregnane X receptor, and this induces CYP3A4[8] — the enzyme responsible for metabolising a large share of prescription medicines. Induction means the drug is cleared faster, so blood levels fall.
It also appears to induce CYP2C9, which is how it can diminish warfarin anticoagulation[9].
A review summarising the field noted that deleterious effects are most pronounced with anticoagulants, cardiovascular medications, oral hypoglycaemics, and antiretrovirals[6].
Enzyme induction is a state, not an event. Spacing doses by four hours does nothing, because the liver’s clearance capacity has been altered for as long as the supplement continues.
Inhibition works the other way and matters just as much. CYP3A4 inhibitors raise blood levels of drugs cleared by that pathway. Grapefruit is the famous one, but the list also includes pomegranate, ginger and turmeric[10] — all of which appear in supplement form and are widely assumed to be inert.
What this means in practice
Three questions, matched to the three mechanisms.
Does it bind? If so, separate the doses. Four hours is the standard interval for levothyroxine and minerals. This is the easy category.
Does it change metabolism? If so, timing is irrelevant — the question is whether to take it at all, and that belongs with the prescriber. This category includes most herbal supplements.
Does it push the same way as the drug? Anything affecting bleeding, blood pressure or blood sugar deserves checking against medication doing similar work.
And one practical habit that covers all three: bring the actual containers, or photographs of the labels, to appointments. A list written from memory tends to omit the multivitamin, and the multivitamin is often where the interaction lives.
If you’re in Korea
Two things specific to here.
Vitamins and minerals are sold as over-the-counter medicines as well as health functional foods. Someone taking a pharmacy vitamin product may not think of it as a supplement at all, which means it doesn’t get mentioned. For interaction purposes the category is irrelevant — what matters is what’s in it.
Pharmacists are accessible and a good first stop. Bringing your containers to a pharmacy for a check costs nothing and doesn’t require an appointment. If you’re taking any prescribed medication alongside supplements, that conversation is worth having before adding anything new.
Closing
What I’d been carrying was a vague sense that supplements and medicines shouldn’t be taken together, without knowing why or what to do about it.
The three-mechanism framing turned out to be more useful than any list. Binding is a timing problem with a timing solution. Metabolic interference isn’t fixable by scheduling. Additive effects need a conversation about the combination itself.
And the vitamin K case corrected something I had genuinely wrong. The instruction is consistency, not avoidance — which is a different behaviour, and one that a “things to avoid” list would never have conveyed.
